1、XXX小學(xué)績(jī)效量化考核細(xì)則一、指導(dǎo)思想1、全面貫徹國(guó)家教育方針,進(jìn)一步推行素質(zhì)教育,切實(shí)提高教育教學(xué)質(zhì)量。2、加強(qiáng)師德師風(fēng)的考核力度,突出教學(xué)工作的中心地位。3、注重目標(biāo)落實(shí),分類量化,突出重點(diǎn),實(shí)施績(jī)效兌現(xiàn)。4、強(qiáng)化過(guò)程管理,以人為本,考核分作教師評(píng)優(yōu)評(píng)先重要依據(jù)。5、加強(qiáng)教師責(zé)任感,提升整體形象,與時(shí)俱進(jìn),推動(dòng)學(xué)校全面發(fā)展。二、基本原則1、實(shí)事求是、公平公開,優(yōu)質(zhì)優(yōu)分、多勞多得。2、定期公布量化結(jié)果,每學(xué)期末匯總,統(tǒng)一結(jié)算發(fā)放。三、績(jī)效考核辦法:教師績(jī)效考核工資,按省市縣文件精神執(zhí)行。把教師績(jī)效工資拿出來(lái)作為教師激勵(lì)性獎(jiǎng)勵(lì)。四、考核內(nèi)容、分值及分工 (合計(jì)100分)1、師德師風(fēng)5分2、教師
2、考勤17分3、教學(xué)工作。53分 由教導(dǎo)處負(fù)責(zé)考核。(1)課堂常規(guī)28分(2)教學(xué)教研10分(3)教學(xué)六認(rèn)真15分4、教學(xué)成果。10分 由教導(dǎo)處負(fù)責(zé)考核5、安全工作。15分6、衛(wèi)生工作(加分項(xiàng))五、分類量化考核細(xì)則(一)、師德師風(fēng)(5分)(違反教師職業(yè)道德和違反法律法規(guī)、綜合治理規(guī)章實(shí)施一票否決)1、參加政治學(xué)習(xí)(2分):積極加強(qiáng)理論學(xué)習(xí),保持高尚的職業(yè)道德。完成學(xué)習(xí)筆記,積極上繳繼續(xù)教育培訓(xùn)手冊(cè),參加校本教研。沒(méi)完成者一次扣2分,不參加活動(dòng)者一次扣1分,不按時(shí)完成者一次扣0.5分。累計(jì)扣分。2、堅(jiān)持家訪制度(1分):緊密聯(lián)系家長(zhǎng),注意及時(shí)反饋,并作好記載。每學(xué)期家訪達(dá)5次及以上,學(xué)校以檢查文字
3、記載為準(zhǔn)。缺少一次扣0.25分。3、強(qiáng)化個(gè)人素質(zhì)(0.5分):進(jìn)教室不穿拖鞋,不穿背心短褲。坐班不做其他事、不得大聲喧嘩與講與教育教學(xué)無(wú)關(guān)的話題,發(fā)現(xiàn)違反一次扣0.25分。累計(jì)扣分。4、注重和諧發(fā)展(0.5分):服從學(xué)校領(lǐng)導(dǎo)安排,教師間互相尊重,團(tuán)結(jié)協(xié)作,嚴(yán)于律己,寬以待人,處理好同事之間的關(guān)系。出現(xiàn)一次糾紛扣雙方當(dāng)事人每人0.5分。5、尊重全體學(xué)生(1分):維護(hù)學(xué)校聲譽(yù),公平對(duì)待學(xué)生,不歧視、體罰、變相體罰學(xué)生,不許驅(qū)趕學(xué)生出教室,造成影響,出現(xiàn)一定后果者,發(fā)現(xiàn)一次扣0.5分。(二)、教師考勤(17分)(一周一統(tǒng)計(jì)、一月一公布)6、上課(3分):教師無(wú)故曠教一節(jié)扣2分,出現(xiàn)嚴(yán)重后果由個(gè)人直接
4、負(fù)責(zé),師德一票否決;自由調(diào)整課程內(nèi)容,課堂呈現(xiàn)放鴨式教學(xué)者,室外課師生分離等發(fā)現(xiàn)一次扣1分。7、簽到(10分),學(xué)校嚴(yán)格執(zhí)行簽到制度,不準(zhǔn)代簽、預(yù)簽、不簽,出勤一次加1分,簽到時(shí)間為上下午大預(yù)備與小預(yù)備之間。8、嚴(yán)格執(zhí)行請(qǐng)銷假制度,請(qǐng)假必須履行請(qǐng)假手續(xù)。病假憑醫(yī)院證明。教職工直系親屬婚、喪假學(xué)校批假四天。沒(méi)有履行請(qǐng)假手續(xù),視為缺勤,一次扣0.5分,一周內(nèi)請(qǐng)假三次不扣分,超過(guò)三次,每次扣0.5分。9、升旗(1分):堅(jiān)持參加升國(guó)旗儀式,做到準(zhǔn)時(shí)、安靜、嚴(yán)肅,并行注目禮,缺席一次扣0.5分,累計(jì)扣分。10、會(huì)議(3分):教師必須按時(shí)參加學(xué)校及各線組織的大小會(huì)議。遲到、早退(超5分鐘)扣0.5分,曠會(huì)
5、一次扣2分。累計(jì)扣分。附:學(xué)期無(wú)請(qǐng)假、遲到、早退、曠勤、調(diào)課等現(xiàn)象計(jì)全勤嘉獎(jiǎng)分3分。 請(qǐng)假手續(xù):、教師個(gè)人請(qǐng)假先調(diào)整好課程后告知值日領(lǐng)導(dǎo)記載。、請(qǐng)假一天內(nèi)由值日領(lǐng)導(dǎo)批準(zhǔn),一天以上報(bào)請(qǐng)校長(zhǎng)批準(zhǔn),一周或一周以上由中心學(xué)校批準(zhǔn)。、公差調(diào)課由教導(dǎo)處協(xié)調(diào)處理。、教師由他人口頭傳話視作無(wú)效請(qǐng)假。(三)教育教學(xué) (53分)(1)課堂常規(guī)(28分)11、上課準(zhǔn)備(2分):教師不許空手進(jìn)入課堂,必須帶齊課本與備好的教案。教師不帶教案進(jìn)入教室上課發(fā)現(xiàn)一次扣0.5分。12、關(guān)愛學(xué)生(1分):上課前必須清查本班學(xué)生,對(duì)遲到、早退,曠課等異常情況要查問(wèn)并做好記載且向班主任反映情況,協(xié)助班級(jí)管理。對(duì)漠然視之者發(fā)現(xiàn)一次扣0
6、.25分。13、課堂紀(jì)律(2分):強(qiáng)化課堂秩序,注重教學(xué)效果,加強(qiáng)班級(jí)建設(shè)。對(duì)講小話和打瞌睡的學(xué)生要堅(jiān)決制止。禁止不力發(fā)現(xiàn)一次扣0.25分,放任自流,堅(jiān)決杜絕教師隨便出教室,發(fā)現(xiàn)一次扣0.5分。14、多媒體教學(xué)(1分):利用遠(yuǎn)程資源,加強(qiáng)業(yè)務(wù)自修,按要求帶領(lǐng)學(xué)生到電教室上課,并完成相關(guān)資料。15、課時(shí)(17分)教師平均周課時(shí)為17課時(shí),達(dá)標(biāo)者記17分。學(xué)校領(lǐng)導(dǎo)因公事課時(shí)沒(méi)達(dá)標(biāo)者作平均課時(shí)記載。16、監(jiān)考閱卷(5分):學(xué)校每學(xué)期組織的期中期末兩次考試,參加監(jiān)考閱卷分別記5分(有試卷、成績(jī)單、試卷分析),缺少一項(xiàng)扣1分,單元測(cè)試(加分項(xiàng)),每測(cè)試一單元加2分,有試卷,成績(jī)單。(2)教學(xué)教研 (10
7、分)17、能認(rèn)真按時(shí)完成布置的教學(xué)計(jì)劃、總結(jié)和論文等文字材料(5分)。一次不完成扣1分。18、在各級(jí)別正式刊物上發(fā)表論文、報(bào)道,按市、地、省、國(guó)錄用分別記1分、1.5分、2分、3分(同篇取最高獎(jiǎng)記分)。(憑原件)19、服從學(xué)校安排、勇于主講公開課,授課教師加4分,聽課、評(píng)課加2分。20、能積極參加上級(jí)組織的優(yōu)質(zhì)課比武,獲縣級(jí)獎(jiǎng)記5分,市級(jí)獎(jiǎng)記10分。21、凡本學(xué)期被評(píng)為市、縣以上教學(xué)能手或骨干教師分別記10分、5分。(3)教學(xué)六認(rèn)真(15分)22、關(guān)于備課、教案(5分):各學(xué)科要按要求備完所有內(nèi)容,教案編寫認(rèn)真,不缺項(xiàng),超課時(shí)一周,上級(jí)檢查出一處錯(cuò)誤扣1分,扣完為止。23、作業(yè)批改(6分):檢
8、測(cè)與作業(yè)要按要求完成規(guī)定的量次。批改要求:文科:要對(duì)錯(cuò)字,錯(cuò)句更正,作文批改要有眉批、尾批。理科:不許以“”代改,由老師橫線指出且由學(xué)生在作業(yè)本上更正,作業(yè)應(yīng)有評(píng)定等次和批改時(shí)間。批改不合格被檢查出一處扣0.5分,量次缺少一次扣0.2分,累計(jì)扣分(扣完為止)。文科、理科作業(yè)本均有訂正。24、聽課筆記(2分):每學(xué)期教師完成8節(jié)聽課任務(wù),少一節(jié)扣0.5分。(扣完為止)25、上級(jí)迎檢(2分):在縣、鎮(zhèn)檢查工作中,對(duì)教師各項(xiàng)工作提出表?yè)P(yáng)的加20分,提出批評(píng)或質(zhì)疑者一次扣10分。(四)教學(xué)成果(10分)26、基礎(chǔ)分5分27、在鎮(zhèn)級(jí)舉行的語(yǔ)文數(shù)學(xué)知識(shí)檢測(cè)競(jìng)賽中獲前六名者分別獎(jiǎng)600元、500元、400元
9、、300元、200元、100元??荚嚦煽?jī)與上期相比一、二、三年級(jí)每提高一個(gè)位次獎(jiǎng)10元,四、五、六年級(jí)每提高一個(gè)位次獎(jiǎng)15元。在學(xué)校舉行的期中、期末考試中獲得前三名者分別獎(jiǎng)100元、80元、60元。(五)安全工作(15分)28、安全工作實(shí)行一票否決制,教師所任班級(jí)發(fā)生一起重大安全事故,此項(xiàng)不得分。(六)、衛(wèi)生工作(加分項(xiàng))學(xué)校每周二下午舉行衛(wèi)生大掃除,班級(jí)評(píng)出一、二、三等獎(jiǎng),任課教師每參加一次加2分。29、計(jì)算公式:總績(jī)效工資總績(jī)效分個(gè)人得分=個(gè)人績(jī)效工資此方案在本學(xué)期開始試行,在實(shí)施中將逐步完善。其解釋權(quán)屬校委會(huì)QcWA3PtGZ7R4I30kA1DkaGhn3XtKknBYCUDxqA7F
